Elevation Change and Effective Distance Info Added

The Dist & Par page and the pages for individual holes have been revised to add information about the change in elevation from tee to basket and the "Effective Distance" for each hole. Per PDGA guidelines, the "Effective Distance" of a hole is the sum of the distance from tee to basket plus three times the change in elevation from tee to basket. Effective Distance is intended to be a rough measure of how long a hole would play if the hole had no elevation change. In addition, the pages for the individual holes have been redesigned so that there is now a single page for each hole that has the basic hole information, a hole description and a scrollable image gallery for the hole, rather than three separate pages for hole information, hole images and a hole map.
